The only other possible causes would be:
Broken cut-off bar
Broken switch (the rail where the cut-off bar rides)
Cut-off bar not being engaged by the sector gear
Damaged sector gear
External selector lever not putting the selector plate in proper position (ruled out?)
These are the only things I can think of. You may have to open up the gearbox again.
