The arm is pressed onto the axel...take a close look to see if it's not sitting where it should be (usually comparing to a "known-good" one is best but on one I came across it was evident).
One I had on a repair, the arm was not in position. It's like the axel had be hammered out a bit. The longitudinal ridges on the axel were visible below the arm (as it sits in the mechbox)...and were binding in the hole.
I knocked it back into position and it's been fine ever since.
Aside from ones where the tip has been damaged, or else the spring has been mangled...that's the only time I've had one not work.
